Nail Surgery in Podiatry: A Permanent Solution for Problem Nails

What Is Nail Surgery?

Nail surgery is a simple, minor procedure performed by podiatrists to treat persistent or painful nail problems — most commonly ingrown toenails that haven’t responded to conservative treatment. The procedure is usually done under local anaesthetic and aims to provide long-term relief.

Whether you’re dealing with recurring infections, nail deformities, or thickened nails causing discomfort, nail surgery can be a safe and effective option.

Common Reasons for Nail Surgery

Chronic ingrown toenails
Repeated infections
Pain from thickened or deformed nails
Involuted (curved) nails
Nail trauma or damage

When conservative care such as trimming, footwear changes, or antibiotics fails, surgery may be the best next step.

Types of Nail Surgery

We offer two main approaches.

1. Partial Nail Avulsion (PNA)

  • Only the edge of the nail growing into the skin is removed.
  • A chemical (usually phenol) is applied to the nail root to prevent regrowth in that section.
  • This is the most common and effective procedure for ingrown nails.

2. Total Nail Avulsion (TNA)

  • The entire nail is removed when it’s severely damaged or diseased.
  • This may also be followed by phenolisation to prevent the nail from regrowing.

What to Expect During the Procedure

  • The toe is numbed using local anaesthetic.
  • The nail edge or full nail is gently removed.
  • Phenol is applied (if permanent removal is needed).
  • A sterile dressing is placed, and you can usually walk out of the clinic with minimal discomfort.

Aftercare and Recovery

  • We provide detailed aftercare instructions and follow-up appointments.
  • You may experience mild soreness for 1–2 days.
  • Most patients can return to normal shoes and activities within a few days.
  • Healing typically occurs within 2–4 weeks.

Is Nail Surgery Painful?

The procedure itself is not painful due to the local anaesthetic.

Some discomfort may occur once the anaesthetic wears off, but this is usually mild and short-lived. Over-the-counter pain relief is often sufficient.

Is Nail Surgery Right for Me?

If you’ve had multiple infections, need frequent nail care, or are in pain despite trying conservative treatments, nail surgery might be the best solution. Our podiatrists will assess your condition, discuss your options, and help you make an informed decision.
