Medicare
Chronic Condition Management Plan (CCMP)
All appointment fees are required to be paid at the time of the consult. If you have been provided a Chronic Conditional Management Plan (CCMP) the rebate will be processed by our practice management software and Medicare will return the rebate into your chosen account.
DVA
TAC/Workcover
Patients under TAC or Workcover will need a letter of referral from their general practitioner stating the service that they require.
Out of pocket fees apply.
Cancellation Policy
We understand you may need to cancel or reschedule your appointment from time to time, however to ensure we have enough notice to allow other patients to be treated and to avoid a cancellation fee please give 24 hours notice.
Missed appointments or less than 24 hours notice will be charged fee.
FEE APPLICABLE is half of the appointment fee.
