Chronic Condition Management Plan (CCMP)
General foot care appointments under Chronic Condition Management Plan (CCMP) with LIA and JAMES booked between 11am and 1pm, or standard consult fee will apply.

Medicare rebates applicable, this excludes aids/equipment such as orthotics/ CAM walkers and certain specialist therapies such as shockwave therapy.
DVA
TAC/Workcover
Patients under TAC or Workcover will need a letter of referral from their general practitioner stating the service that they require.

Cancellation Policy
We understand you may need to cancel or reschedule your appointment from time to time, however to ensure we have enough notice to allow other patients to be treated and to avoid a cancellation fee please give 24 hours notice.
Missed appointments or less than 24 hours notice will be charged fee.
FEE APPLICABLE is half of the appointment fee.
