A fifth metatarsal avulsion fracture typically occurs with an inversion injury. The sudden pull of the peroneus brevis muscles that occurs when rolling you ankle results in the fracture of the fifth metatarsal.
An avulsion fracture can also occur with a sudden force applied to the 5th metatarsal bone.
